Summary:

The Certified Diabetes Care and Education Specialist (CDCES) is a skilled and professional registered dietitian or registered nurse with competence in diabetes and lifestyle medicine. This role works cooperatively and communicates effectively within a multidisciplinary framework to actively promote holistic care for patients and families. This position demonstrates self-direction, excellent problem-solving ability, and initiative in the role of educator, care and support specialist. Responsible for providing individualized diabetes self-management education/training and medical nutrition therapy by a Registered Dietitian, integrated with the patient\'s medical goals to individuals and groups according to the Scope of Practice, Standards of Practice, and Standards of Professional performance for Diabetes Educators (AADE, 2011).

Duties and Responsibilities include the following: * Collects DSMES program participant assessment data in a collaborative and ongoing manner. *

  • Collaboratively develops educational goals, learning objectives and a plan for educational content and teaching methods with DSMES program participants. *
  • Provides educational interventions that utilize primarily interactive, collaborative, skill-based training methods and maximizes the use of interactive training methods. *
  • For Registered Dietitians, coordinate all phases of nutrition care including nutrition assessment, care planning, monitoring, education, and counseling. *
  • Collaboratively develops an individualized follow-up plan with each program participant.*
  • Evaluates effectiveness of educational services provided by measuring attainment of learning objectives. *
  • Conducts a follow-up assessment upon completion of DSMES program services, using outcome measures. *
  • Documents assessment data, educational plan, educational services provided and evaluation results in each participant\'s educational records. *
  • Utilizes a team approach to provide services and collaborates and communicates with team members when needed. *
  • Identifies when a program participant\'s needs are outside the scope of the instructor\'s practice and expertise, plus arranges for additional services to meet needs. *
  • Participates in the development of training materials. *
  • Contributes to, and participates in, a continuous quality improvement process to measure DSMES program and to identify and address opportunities for improvement. *
  • Appraises his/her performance to identify areas of strength and areas for improvement and to develop a plan for improvement and growth. *
  • Participates in peer review process to evaluate performance of other professional instructional staff. *
  • Participates in planning and developing nutrition and diabetes management related policies, procedures, and goals. *
  • Participates in programs to meet continuing education requirements. *
  • Responsible for occasional evening or weekend work as needed.
  • Performs other duties as assigned.

POSITION: Certified Diabetes Care and Education Specialist - RD/RN

POSITION REQUIREMENTS: Education/Experience:

Certified Diabetes Care and Education Specialist (CDCES) with at least three years of experience in diabetes care, education, and knowledge in program management.

Registered Dietitian/Registered Dietitian Nutritionist with active registration by the Commission on Dietetic Registration (CDR) of the Academy of Nutrition and Dietetics (AND) and certified by NY State as Certified Dietitian Nutritionist (CDN) or Registered Nurse with active NY State License.

Experience with insulin pumps and continuous glucose monitoring technology preferred

Prior experience in DSMES Program management with ADA ERP or ADCES Education Program preferred.

Certificates and Licenses:
  • Certified Diabetes Care and Education Specialist (CDCES) in good and current standing with the Certification Board for Diabetes Care & Education (CBDCE)
  • Registered Dietitian Nutritionist (RD/RDN)in good and current standing with the Academy of Nutrition & Dietetics (AND), and the Commission on Dietetic Registration (CDR)
  • Certification/Licensure as a Certified Dietitian Nutritionist (CDN) in New York State
  • Registered Professional Nurse with active NYS licensure and a minimum of 3 years of experience in diabetes education and management
