Job Summary

We are hiring for Elkhart, Lagrange, Noble and Marshall counties.

We are seeking a dedicated, compassionate Registered Nurse (RN) or Licensed Practical Nurse (LPN) with strong independent assessment skills to join our healthcare team. The ideal candidate will provide high-quality patient care in the home care setting of Amish communities of Northern Indiana.

These are acute care Telehealth visits approximately 30 min per visit. You would conduct the assessment and be able to communicate well with the patient, family and Practitioner. Training provided.

Previous home care experience preferred, but not necessary if you possess strong clinical assessment skills. These visits include both pediatric and adult patients. Electronic charting.

Duties

Administer medications and treatments as prescribed by practitioner. Monitor patient vital signs and report to the healthcare team. Complete focused patient assessments and communicate effectively. Maintain accurate medical documentation and records in compliance with HIPAA regulations. Collaborate with members of the team to ensure comprehensive patient care. Educate patients and their families on health management and wellness strategies. IV and venipuncture skills a plus.

Skills

Candidates should possess a variety of skills that enhance their ability to provide exceptional patient care. Preferred skills include:

Willingness to learn EMR: Optimantra 3 years experience in hospital or outpatient setting. Knowledge of medical terminology, anatomy, and physiology. Skills in IV infusion, phlebotomy, medication administration, and basic life support. Experience working with diverse populations including geriatrics and pediatrics. Competence in performing diagnostic evaluations and physical examinations. Understanding of infection control practices and aseptic technique. Valid Drivers license.
