Job Summary

The RN Navigator is responsible for working with the Quality Team to maintain accreditation for Joint Commission Certified Program in Advance Total Hip and Knee Program, and to achieve Joint Commission Certification for programs in Neuro Spine, Bariatrics and other clinical specific programs during a timeframe developed.

Under the direction of the Director of Quality and Patient Safety, the RN Navigator will function as the expert in disease specific clinical practice, patient education, research, both data and patient management, problem-solving as well as the coordinator of care throughout the care continuum. The RN Navigator will focus on patient-centered care, collecting, tracking, trending, and analyzing data for quality improvement.

Key Responsibilities * Facilitate and coordinate care for patients undergoing procedures in our programs supported by the incumbent to achieve optimal clinical, financial, operational and satisfaction outcomes.

  • Collect, track, trend, and analyze program outcome data for required submission to TJC and hospital committees
  • Provide necessary information and education to the patient for preoperative, intra-operative, recovery, discharge, and post discharge processes
  • Develops and maintains protocols, policies, and procedures in collaboration with interdisciplinary team using current evidence-based guidelines to ensure positive patient outcomes
Minimum Qualifications * Education: Bachelor\xe2\x80\x99s Degree in Nursing required
  • Experience: Minimum of 3 years-experience as a Massachusetts Registered Nurse in a hospital setting with 2 years of applicable experience preferred.
  • Certification/Licensure: MA Nursing License, BLS required. Clinical Certification in a Specialty Practice Area Preferred.
  • Software/Hardware: Strong computer proficiency required, including Meditech, Microsoft Office products, Databases, and Internet.
  • Other: Must demonstrate excellent organization and multitasking skills. Must demonstrate professionalism and possess the skill, knowledge, and ability to interact collaboratively with healthcare teams. Demonstrates innovation, expert problem-solving skills and is self-motivated.
