Overview

The RN Field Supervisor is a necessary position ensure that our clients receive
complete, safe, and appropriate services in their home. Depending on the state, this
may be a mandated position required to provide care.
Responsibilities

\xe2\x97\x8f This position is responsible for the overall health and safety of the client
\xe2\x97\x8f The RN Field Supervisor is responsible for compliance with the rules and regulations related to his/her duties.
\xe2\x97\x8f In conjunction with the Branch Manager, the RN Field Supervisor may complete initial Assessment visits for pending referrals/admissions.
\xe2\x97\x8f Evaluate clients referred for home care to determine appropriateness for admission, determine safety of environment
\xe2\x97\x8f Be an advocate for clients needing other community services
\xe2\x97\x8f Coordinate with family, physicians and other disciplines as needed when establishing plan of care
\xe2\x97\x8f Perform initial assessments, initiate plan of care, review plan of care every 90 days (making changes according to client needs)
\xe2\x97\x8f Assist in reviewing biweekly timesheets
\xe2\x97\x8f Perform competency review during orientation and on-going with supervision
\xe2\x97\x8f Determine correct staff (PCA vs CNA) is assigned to client according to client\xe2\x80\x99s levels of assistance need
\xe2\x97\x8f Be on site during any state survey/audit as requested, also be present for exit interview
\xe2\x97\x8f The RN Field Supervisor is responsible for ongoing quarterly supervisory visits within the established, required timeframe.
\xe2\x97\x8f Provide competency evaluations for new/current employees
\xe2\x97\x8f Assist the Branch Manager, serve as back-up, as necessary.
\xe2\x97\x8f On-Call, as appropriate

\xe2\x97\x8f In conjunction with the Branch Manager, review complaints, concerns, and incidents and develop an internal Plan of Correction to prevent future incidents.
\xe2\x97\x8f Work with Staffing Coordinator and Branch Manager to develop ideas to improve recruitment and retention of attendants for the office.
\xe2\x97\x8f Assist in determining the appropriate action around the discharge of a client that the office is initiating.
\xe2\x97\x8f Enter authorizations for clients, as assigned
\xe2\x97\x8f Participates in scheduled meetings
\xe2\x97\x8f Other duties, as assigned.
Qualifications

\xe2\x97\x8f Have an active, unrestricted Registered Nurse license in Rhode Island or an alternate compact state, Bilingal RN a plus
\xe2\x97\x8f Experience in Home and Community Based Services, preferred.
\xe2\x97\x8f Excellent oral and written communication and presentation skills.
\xe2\x97\x8f Professional demeanor and appearance.
\xe2\x97\x8f Proficient skills to promote excellent client relations and customer service.
\xe2\x97\x8f Excellent organizational and time management skills
\xe2\x97\x8f Have reliable transportation
