Summary of Responsibilities:




The Residential Medical Appointment Facilitator & Discharge Planner (RP) is tasked with providing coordination/transportation and scheduling of medical appointments and discharge planning for Residential Program students.

  • Works collaboratively with the Residential nursing department, direct care staff and community providers the RP takes lead in establishing and facilitating all medical and related appointments.
  • Works collaboratively with students and families in the development of discharge plans for Residential Program students post placement under the direction of Residential Clinical Therapists.
  • Ensures fluid and dynamic utilization of agency and community resources to assure students are having their medical needs and discharge planning provided.
  • Establishes, confirms and as needed transports the medical necessary appointments, consultations, evaluations and treatments, follow up or reevaluation for students while in Residential care.
  • Works within the electronic health record and email systems to assure documentation regarding appointments and follow ups are clearly communicated and data is accurately collected.
  • Collaborates and proactively creates plans for student discharges including but not limited to attending plan of care meetings prior to discharge, organizing and submitting post-discharge referral services, providing access for discharge placements and clearly enrolling student/family in outpatient services.
  • Communicates with appropriate team members regarding the needs of the appointments and discharge plans.

All employees with AK Child & Family are required to be in compliance with the Centers for Medicare and Medicaid COVID vaccine mandate. The mandate requires that by 2/14/22, staff at all health care facilities must have received their first dose of a 2-shot series (Moderna or Pfizer, currently) or a single dose of a 1-shot vaccine (Johnson and Johnson, currently) with the exception of those that have sought and been granted an exemption to the COVID mandate. Staff must complete this step before they can provide any care, treatment or other services for the facility and/or its patients. By 3/15/22, all staff with the exception of those that have sought and been granted an exemption to the COVID mandate, must complete the primary vaccination series.

Pay:
  • $17.32 + DOE

Supervision and Coordination of Services:

  • The Residential Medical Appointment Facilitator & Discharge Planner (RP) reports to the Clinical Residential Supervisor.
  • S(he) works collaboratively with all agency personnel, particularly Treatment Program Supervisors, Clinical staff, Residential Behavior Health Specialists and the especially the Nursing Dept. adhering to agency mission, core value, policies, and Sanctuary principles.





Qualifications:



Knowledge, Skills and Abilities:
  • The Residential Medical Appointment Facilitator & Discharge Planner (RP) must have demonstrated organizational and detail-oriented skills.
  • The ability to manage competing priorities and problem solving abilities required.
  • S/he must be detail oriented, able to work independently and with a team, able to review and examine documentation for thoroughness and appropriately convey information orally and in writing with clients, providers and the clinical team.
  • Knowledge of electronic health record systems and documentation skills a must.
  • Previous experience working with severely emotionally disturbed youth is preferred but not required.
  • The ability to communicate effectively verbally and in writing is essential.
  • Demonstrated skill in scheduling, communication and transports is essential with prior experience preferable.

Education and Experience
The Residential Medical Appointment Facilitator & Discharge Planner (RP) must meet one of the 3 requirements below:
  • (1) a bachelor\'s level degree program from an accredited college or university in a human services related field;
  • (2) At least six months of full-time work experience in residential treatment for children, inpatient long-term treatment for children, or other treatment of severely emotionally disturbed children and an associate level degree from an accredited college or university in a human services related field; or at least two years of college credit toward a bachelor\'s degree from an accredited college or university in a human services related field;
  • (3) At least one year of full-time caregiver experience in residential treatment for children, inpatient long-term treatment of children, or treatment of severely emotionally disturbed children and a high school diploma or GED





Physical Demands:


  • The Residential Medical Appointment Facilitator & Discharge Planner (RP) must have the ability to use a personal computer.
  • They must be able to work on a copy machine forty-four (44) inches high and to file in cabinets up to thirty-five inches high.
  • The Residential Medical Scheduler & Discharge Planner must be able to use his/her own vehicle and/or an agency vehicle in order to attend/participate in meetings and provide transportation for clients.
