This role includes being listed as the supervising RN Allied Support LLC is a licensed Comprehensive Home Care provider in Minnesota and Skilled Nursing. Our team is dedicated to high-quality, client-centered care while maintaining full compliance and we need someone dedicated and proficient to help people in a on our home care license and providing client-focused nursing services such as PCA supervision.

Responsibilities

Conduct thorough patient assessments to identify health issues and needs.

Tracheostomy care ( 64 HOURS PER MONTH)

Conduct PCA supervision visits (every 90 days or as needed). Provide medication setup, reconciliation, and oversight. Complete client assessments and update care plans. Document care in compliance with MDH standards. Communicate with the care team, case managers, and families as needed. Monitor patients' vital signs and report any significant changes to the healthcare team. Maintain accurate patient records and documentation in compliance with healthcare regulations. Provide

comprehensive tracheostomy care

, including suctioning, tube changes, stoma care, and emergency tracheostomy management. Monitor and assess respiratory status, oxygen saturation, and overall patient condition. Administer medications and treatments as prescribed. Educate residents and families on tracheostomy care and respiratory hygiene as appropriate. Maintain accurate documentation in accordance with facility policy and state/federal regulations. Follow all infection control procedures and safety guidelines established by the

Minnesota Department of Health (MDH)

and

Centers for Medicare & Medicaid Services (CMS)

. Participate in ongoing training and professional development to maintain competency in tracheostomy and skilled nursing care.
