Position Summary:

We are currently seeking a

Registered Nurse (RN)

to join our caring team. The RN will coordinate care plans and ensure that each client receives the highest standard of home health services.

Key Responsibilities:

Conduct initial and ongoing patient assessments in the home setting. Develop and implement individualized care plans. Collaborate with Caregivers and Office Staff teams. Educate patients and families on Home Health management and care practices. Document all nursing activities in compliance with agency and state regulations.

Qualifications:

Active

Registered Nurse (RN) License

in Rhode Island. Minimum

1 year of clinical experience

, preferably in home care or community health. Strong communication, organizational, and assessment skills. Compassionate and patient-centered approach. Reliable transportation and valid driver's license. Bilingual (English/Spanish) candidates are highly valued
