The Quality Outcomes Specialist position is responsible for assessing and facilitating clinical systems and/or processes to ensure that care delivered is safe, effective, patient-centered, timely, efficient and equitable. The Quality Outcome Specialist in collaboration with clinical and operational leaders is responsible for developing, implementing, and overseeing quality improvement initiatives across the health system. This role focuses on enhancing patient outcomes, ensuring regulatory compliance, and promoting a culture of continuous improvement in hospitals, clinics, post-acute care facilities, and home health settings. Other responsibilities include serving as departmental expert for areas of expertise, sharing knowledge, effective tools and educational materials as appropriate.

Key Accountabilities



Lead and coordinate quality improvement projects using methodologies such as Six Sigma, Lean, and PDSA cycles Develop and implement strategies to enhance clinical outcomes and patient safety. Collaborate with clinical teams to identify, understand, redesign, and monitor workflows and clinical processes to improve efficiency and effectiveness. Implement evidence-based practices and clinical guidelines to standardize care delivery Facilitates implementation of initiatives related to quality patient care and population health outcomes that includes but is not limited to risk score accuracy initiatives, pay-for-performance contract special projects, quality care gap closure activities, population outreach and use of population management tools. Facilitates successful provider and care team member orientation to established and new clinical programs. Develop and deliver training programs on quality improvement methodologies and clinical best practices. Provide coaching and mentorship to staff to build competencies in healthcare quality and transformation Collect, analyze and interpret healthcare data to identify trends, gaps and opportunities for improvement and translate into actionable information. Generate reports, dashboards, and scorecards to monitor key performance indicators and quality metrics. Emphasis on using clinical data from various health information technology systems to make informed decisions at the department, practice facility or service line level to improve patient outcome and promote value-based care. Facilitate interdisciplinary meetings and committees to discuss and address quality improvement issues Completes case reviews and supports peer review process when indicated Ensure compliance with healthcare regulations, standards, and accreditation requirements. Prepare for and participate in regulatory and accreditation surveys and audits Maintain continuous knowledge of federal, state and other regulatory standards/regulations, Medical Staff Bylaws and organizational policies and procedures pertaining to quality, safety, case/peer review Supports the RN Quality Data Coordinator in eCQM, process measures, structure measures, and chart abstraction measures Establish and monitor performance benchmarks through research against industry standards and best practices. Coordinates focus studies and researches medical literature when necessary to evaluate quality measures, benchmarks and quality of care. Utilize electronic health records and health information systems to support quality improvement efforts Ensure data integrity and security in all quality and clinical transformation activities. Works closely with the analytics team to obtain actionable information to improve patient care and service, improve population health management, and reduce the total cost of care. Promote individual professional growth and development and serves as preceptor, mentor and resource to less experienced staff.

Qualifications



Minimum of 3 to 5 years of clinical care experience in area of intended practice and process improvement (acute, ambulatory, post-acute). Experience with and/or working knowledge of basic process improvement method: PDSA, Process mapping, and project management is required. Experience in performance improvement activities is preferred. Prior leadership experience preferred. Bachelor of Science Degree in Nursing or other healthcare related bachelor's required. Master of Science in Nursing or other healthcare related master's degree preferred. Knowledge and experience with a wide variety of computer programs including Word, PowerPoint, Excel and Outlook. Must be proficient in EPIC electronic health record Advanced training in process improvement methodologies including: Lean, Six Sigma and Change Management. Certified Professional in Healthcare Quality (CPHQ) by the National Associate for Healthcare Quality strongly preferred or ability to obtain within 18 months of start date. Licensed RN in WI, a compact state, or ability to become licensed in WI

Excellent verbal and writing skills Advanced interpersonal communication skills for extensive interactions with diverse healthcare professionals including nurses, physicians, administrators, department leaders and team members to implement quality initiatives across the care continuum, and provide effective consultation and collaboration Ability to organize and define problems, develop strategies, and carry out action plans Advanced strong analytical skills to interpret, trend and analyze complex medical data and patient outcomes A strong understanding of clinical practices and patient care pathways is often necessary to effectively evaluate quality issues. Capable of exercising sound judgment and making independent decisions Ability to adapt to multiple complex organizational environments Strong communication skills with the ability to build relationships with providers, leadership and clinical staff. Comfortable in clinical settings to assist teams at the point of care with problem solving or action plans related to RCAs. Presentation skills and ability to adapt/communicate relevant message to audience. Excellent written and verbal skills. Strong ability to coordinate, lead and execute on team deliverables.

Ability to move freely (standing, stooping, walking, bending, pushing, and pulling) and lift up to a maximum of twenty-five (25) pounds without assistance

Work Environment



25-50% travel expected Normally works in climate-controlled office environment Frequent sitting with movement throughout office space and departments/clinics Use of computers throughout work day Frequent use of keyboard with repetitive motion of hands, wrist and fingers Interaction with department members and other healthcare providers. When rounding, possible exposure to communicable diseases, hazardous materials, and pharmacological agents.
