Job Description

The Program Specialist plays a key role in connecting participants and families to our services and ensuring coordinated, compliant care. This position requires strong communication skills, knowledge of ISP processes, and experience working within the developmental disabilities system.

This role is ideal for someone who enjoys relationship-building, collaboration, and advocating for individuals with developmental disabilities.

Key Responsibilities



Communicate with potential participants and guardians to explain agency services and eligibility Meet with participants and/or guardians in person or virtually Collaborate with Service Coordinators to coordinate services Attend and actively participate in ISP (Individual Support Plan) meetings Assist with developing, writing, and updating ISP goals Ensure services align with participant needs, preferences, and regulatory requirements Maintain accurate and timely documentation Act as a liaison between the agency, families, and coordinating professionals

Qualifications & Experience



Experience working in the Developmental Disabilities field Prior experience working with participants, guardians, and service coordinators Knowledge of ISP processes, goals, and person-centered planning Strong verbal and written communication skills Ability to work independently and manage multiple responsibilities
Preferred (but not required):

Experience with Medicaid-funded DD services Familiarity with state DD regulations and compliance Bachelor's degree in Human Services, Social Work, Psychology, or related field
