Job Summary:
The Program manager performs both supervisory and direct care work. An employee in this classification is responsible for the day-to-day operation of the community home. This position is non-exempt from overtime.
Physical Demands

Physically lift 40-100 pounds unassisted
Physically lift 100 pounds or more assisted
Reach above head and below knees
Endure repetitive motions with right and left hands
Reach out as to pull and push a wheelchair
Make twisting and bending motions at the waist
Have good reaction response to protect personal space
Job Responsibilities
:
Have a working knowledge of all agency policies and procedures and ensure compliance.
Complete 24 hours of training per year, including mandatory trainings and trainings as assigned by your immediate supervisor or administration.
Works in coordination with Program Specialist to ensure all direct care professionals are trained on individual ISP's and individual's care to include but not limited to medical diagnosis, and medications
Ensure that the staffing schedule of the community home is scheduled, posted, and remains current.
Perform direct care hours as assigned
Provide support and problem-solving techniques both with subordinates and with individuals
Schedule & attend required medical appointments with the individuals per regulations and based on the individuals need.
Ensure staff are trained and remain compliant with medical recommendations from medical professionals.
Complete annual performance evaluations with your subordinates.
Use disciplinary procedures as necessary in coordination with human resources and program specialist
Review timesheets, approve timecards, PTO, and attendance requests for subordinates in Futures' approved time and attendance system.
Facilitate staff meetings with Program Specialist for direct support professionals.
Participate and attend agency and individual team meetings.
Act as a liaison between direct support professionals, families, program specialists, and Futures administration.
Ensure compliance with 6100 and 6400 regulations along with updated ODP requirements communicated through bulletins and communications.
Assume responsibility for the general upkeep of the community home, including cleanliness, inventory, funds, medications administration and documentation.
Submit work orders as necessary
Collaborate with Program Specialist to ensure necessary purchases are made for the home in a budget conscious manner.
Perform evening, overnight, weekend, and holiday on-call duties as necessary.
Monitor home to ensure safety procedures are being followed. Be able to identify work safety issues, injuries, and report immediately.
Monitor money coming in and out of the community home, as well as individual's finances.
Assist with maintaining agency vehicles
Facilitate individual supervision with direct care professionals as needed
Have a working knowledge of incident management and ensure incidents are report as required.

Onboard new direct support professionals
Effective and timely communication with immediate supervisor and direct care professionals, families, medical professionals, and all external stakeholders.
Use of positive and respectful written and verbal communication to all team members
Ensure direct care professionals have the appropriate tools and training to effectively support individuals
Build collaborative relationships with all team members
Utilize and model everyday life philosophy in supporting individuals
Complete required documentation in agency approved electronic health record and ensure compliance of agency policies pertaining to documentation and Futures' electronic health record. Review documentation completed by direct support professionals
Completes any and all related tasks as requested by immediate supervisor.
Medical Physical
:
Frontline Supervisors are required to get a pre-employment physical and a physical with a Mantoux TB test every 2 years.
Working Conditions
: Employee in this position may be expected to work evenings, overnights, weekends, and holidays Employee supports individuals on a regular basis who have an intellectual disability and could have co-occurring mental health disorders that present with challenging behaviors including physical and verbal aggression, self-harm, and property destruction. The employee is occasionally exposed to individuals who have the potential to be carrying a communicable disease.
Valid Driver's License
High school diploma
Two years' experience working people with intellectual disabilities preferred
Supervisory experience Preferred
