We are seeking an experienced and results-driven Program Manager to support the growth and daily operations of our Residential Habilitation (ResHab) programs. The primary focus of this role is client acquisition and member intake, ensuring that prospective individuals are identified, enrolled, and smoothly transitioned into services. The ideal candidate will have a strong background in outreach, referral coordination, and relationship-building within the developmental disabilities or human services field.
This position will also oversee the operational performance of our residential programs, ensuring services are delivered in compliance with state requirements and internal quality standards. The Program Manager will collaborate closely with leadership and direct care teams to streamline processes, optimize workflows, and support the continued expansion of our agency.
Responsibilities
Key Responsibilities
Inquiring and filling in member vacancies.
Conducting member intake assessments and reviewing case management and health
record data for new members.
Overseeing daily operations for residential facilities, including supervising House Managers, each of whom have up staff serving members 24/7.
Designing, implementing, and managing evidence-based person-centered policy, services, and programming.
Participating in Individual Support Team (IST) and Positive Centered Plan (PCP) meetings to ensure implementation, review, and necessary modifications of Member Support Plans and Behavior Management Plans.
Evaluating effectiveness of services and programming with emphasis on member feedback to achieve continuous improvement.
Developing and monitoring Member Function Assessment Support Plans and Behavior Management Plans as appropriate.
Document and track incidents
Maintain overall regulatory compliance, including reportable events reporting.
Networking with the case management, healthcare, and social services community and other professionals to ensure that residential facilities operate at capacity.
Experience
Proven experience in client acquisition, outreach, or referral development within a Residential Habilitation, Supported Living, Developmental Disabilities, Home and Community-Based Services (HCBS), or related human services setting.
Demonstrated success managing member intake processes, including eligibility verification, assessments, documentation collection, and coordination with families, case managers, and state agencies.
Background in program operations, preferably in residential or community-based disability services, with a strong understanding of regulatory requirements, service models, and quality standards.
Experience building and maintaining relationships with Support Coordinators, case managers, hospitals, community partners, and other referral sources to generate consistent client leads.
Track record of coordinating and supporting onboarding, placement, and transition of new members into services.
Hands-on experience supervising or supporting direct care staff and overseeing day-to-day program workflows.
Strong skills in project coordination, data tracking, process improvement, and implementing systems that improve efficiency across programs.
Ability to manage multiple priorities, respond quickly to operational needs, and support a fast-paced service environment.
