Position is virtual/remote within New York State only

New York State Based Role.

This role will be the liaison with external agencies to ensure coordination with the services they are providing to MVP Medicaid members. Specifically, as liaison to OCFS, county LDSS, and the 29-I Facility Foster Care Coordinators or Voluntary Foster Care Agencies (VFCA), this role will act as primary contact person to assist with enrollment, disenrollment, and access to care for the foster care children.This professional will collaborate with an interdisciplinary clinical team of MVP Care Managers, Medical Directors and clinicians to ensure timely access to care for children in foster care and ensure smooth care transitions. The role will coordinate with the Health Home Care liaison and the Medically Fragile Liaison as needed.The Foster Care Liaison works with network and operations staff when contracting or claims issues exist.

This position will be responsible for:
  • documenting all work including case tracking and record keeping
  • following guidelines mandated by NYS regulations, corporate policies are followed
  • continued development of workflows as state policy is updated, and
  • ensuring regular, ongoing communications with VFCAs to proactively address their needs.
Performs additional duties as assigned as part of the Medicaid Services and Supports team at MVP.

POSITION QUALIFICATIONS

Minimum Education:
BS required, Master\'s degree preferred

Minimum Experience:
2 years\' experience, expertise and knowledge of the child welfare system, foster care healthcare requirements or Medicaid waiver services for children and the unique complex needs of this population
Experience in health care insurance or managed care preferred
Knowledge of the unique complex needs (including needs resultant of trauma) of children in foster care
Knowledge of NYS Office of Children and Family Services (OCFS) regulations, standards and foster care healthcare requirements
Knowledge of the services for children available in New York State

Required Skills:
  • Experience in Health care
  • Proficient in Microsoft suite.
  • Must have excellent organizational, interpersonal, oral and written communication skills.
  • Must have strong analytical and problem-solving skills.
  • Must be able to work well independently with all levels of staff under limited supervision, be able to prioritize, multi-task while paying attention to details, meet deadlines, and be flexible to adapt to changing organizational needs.
Preferred Skills:

Fluency in Spanish
