Hours: Monday \xe2\x80\x93 Friday | 8:00am \xe2\x80\x93 5:00pm (local time)

About this Role:

The Primary Care Provider at Matter Health is the clinical leader of patient care in the communities we are fortunate to serve. As the leader of patient clinical care, you will be an active and visible member to the community. The Provider will assist residents with the coordination of all their medical needs and social services. Building sincere relationships with your patients and adjusting treatment plans specific to the patient\xe2\x80\x99s needs. You will combine traditional clinical skills with effective community engagement, problem solving and strong organizational skills in a truly rewarding role!

Our Provider is energized by the idea of providing exceptional services to our patients through holistic, evidence-based care. You will conduct physicals, evaluate well-being and devise care plans specific for each member. You are a consistent resource for our members answering any questions they may have, no question is too big or small! The Primary Care Provider will also support the Membership Manager in growing our member base. Whether engaging directly with individuals in the community, or helping to organize events - think fun, informational, engaging and tailored experiences, ice cream socials and BBQs, endless possibilities!

The Day-to-Day:

Patient focused quality and safety

\xc2\xb7 Evaluate primary care, acute, chronically ill, and transitional care patient in addition providing healthcare education and counseling to residents

\xc2\xb7 Provide patient counseling, medication management and education on health maintenance

\xc2\xb7 Document all patient care within Athena, Matter Health\xe2\x80\x99s HER

\xc2\xb7 Provide care and coordination of our patients with internal and external colleagues ensuring the highest standard of care is always provided for all patients

\xc2\xb7 Phone triage outreach, patient education, population health leadership

Patient Experience

\xc2\xb7 Maintain a fixated focus on the overall patient experience

\xc2\xb7 Tailor communication style of effectively influence quality outcomes and building relationships

\xc2\xb7 Maintain patient confidentiality in accordance with HIPAA and PHI

Community Building

\xc2\xb7 Work closely with the Care Navigator and Membership Manager to create a thriving community at the property in which your clinic is located

\xc2\xb7 Work to build personal relationships and social influence with all members of the community

\xc2\xb7 Attend social events and become ingrained in the lives and well-being of each of the patients you serve

Autonomy & Accountability

\xc2\xb7 Work independently in your clinic to prioritize and solve problems

\xc2\xb7 Take initiative to improve your clinic while advocating for your patients

\xc2\xb7 Actively participate in professional development opportunities and groups within your market

\xc2\xb7 Maintain self-awareness and professionalism of individual actions and how they impact the clinic, practice, and healthcare industry

\xc2\xb7 Own the success of your practice through implementation of the clinic level business plan designed to meet identified business goals

Requirements

What Leads to Success?

• Passion for Matter Health model, vision, and patient population
• 3 years of experience as an Nurse Practitioner or Physician Assistant
• A Master\xe2\x80\x99s degree and certification by one of the Nurse Practitioner or Physician Assistant certifying bodies
• DEA certification with no restrictions
• A valid, active and unencumbered state Nurse Practitioner or Physician Assistant license in the State of Tennessee

Nice to Haves:

• A specialization in the field of Gerontology with applicable experience
