Job Summary
The Prevention Programming Coordinator provides direct services to middle and high school-aged youth (ages 12-17) through a combination of school-based mentoring, facilitated group programming, whole-student assessments, diversion services, and family engagement. This role emphasizes youth leadership development by creating opportunities for young people to build skills, strengthen their voices, and actively participate in group discussions and activities. The goal of this position is to prevent involvement in the juvenile justice system, improve school engagement, and strengthen connections between youth, families, schools, and community supports.
Responsibilities of the Prevention Coordinator Include:
Develop meaningful, supportive relationships with youth and their families
Provide mentoring, case management, and short-term solution-focused counseling
Conduct whole-student assessments, including bio/psycho/social assessments
Administer and score assessment tools such as MAYSI-2 and Developmental Assets Profile (DAP)
Provide screening and assessment for youth experiencing challenges at home and/or school
Develop individualized engagement, goal, and school reentry plans
Communicate regularly with referring schools, Human Service Zones, and other referral sources
Coordinate school- and community-based mentoring services
Provide diversion-focused case management to prevent juvenile justice involvement
Offer crisis intervention, stabilization, and goal-setting services
Provide family engagement support, psychoeducation, mediation, and referrals
Maintain accurate and detailed client documentation and case notes
Collect and track attendance, behavior, and engagement data
Support quarterly reporting and grant-related data requirements
Collaborate with community partners, schools, and service providers
Participate in an on-call rotation for after-hours crisis response
Attend staff meetings, trainings, and professional development opportunities
Qualifications
The position requires a four-year degree in a human service field (social work, counseling, education, etc). Preference given to applicants with a master's degree, licensure, and experience working with adolescents and families.
Work Schedule
This is a full-time, benefited position. The typical work schedule is Monday through Friday from 8:30 AM - 5:00 PM. This position requires some evening and weekend work as well as being on the on-call rotation.
Compensation & Benefits
Starting salary range for this position is $45,000, dependent on the education and experience of the applicant. Our benefits package includes medical and dental insurance, life insurance, short and long-term disability insurance, matching 401k plan, vacation and sick leave, paid holidays, and half days on Fridays in the summer.
