Description: Department Overview: As part of Navvis\xe2\x80\x99 Post Acute team you will play an instrumental role in both the quality of care patients receive post discharge and the financial performance derived in value-based contracts by working closely with aligned PAC providers. The Post Acute Network Administrator will drive higher levels of value-based performance with Transitional Care Unit partners through quarterly provider performance scorecard intensive review sessions, problem solving, ease of referral focus and ongoing action planning and tracking. The Administrator will also participate in briefings and education sessions with episode management hospital and physician stakeholders within the local health system, as well as participate in strategic initiatives to advance post acute care into the home. This role is based in the Minneapolis region. As the Post-Acute Care Network Administrator, you will:

  • Regularly review performance High Performing Transitional Care Unit Network through quarterly scorecards (if applicable), ACO Expenditure reports, claims data, and other reporting as available to understand each facility\xe2\x80\x99s performance in Value-Based payment models and other risk-based contracts
  • Meet with each Transitional Care Unit in the High Performing Network intended to create regular channels of interaction, communicate changes, review performance, and understand opportunities for performance improvement while achieving mutually determined goals for both Health System and Post Acute Partners.
  • Identify performance gaps of the High Performing Network overall, as well as for each individual facility, and develop strategies and corresponding action plans to improve performance
  • Lead a team of Transition of Care Managers and following patients in Transitional Care Unit and Post Acute Care programs to ensure proactive discharge planning and care management of patients, identify challenges with facilities or agencies, review specific care examples, and escalate concerns with facility performance as indicated
  • Ensure collaboration between Transition of Care Managers and other care management teams and stakeholders for a streamlined process that allows for the most efficient management of patients
  • Work across Transitional Care Unit and Home health partners and internal stakeholders to advance clinical capabilities to care for an increasingly complex patient mix, including programs such as Transitional Care Unit-at-Home.
  • Meet regularly with Health System Leaders and Value Based Care stakeholders that intersect with Post Acute Performance, such as Hospital leadership, case management, Value based care programs, Health Plan, Geriatric program, etc. to inform them of Home Health and Transitional Care Unit performance, and ensure the network continues to serve the health system and optimize outcomes for our patients
  • Annually, and as indicated, review overall network composition for performance and collaboration and provide recommendations for changes to be made to network membership
  • Provide regular report outs to leadership boards and meetings to allow visibility into Post Acute performance
  • Participate in National PAC Network Administrative meetings quarterly to collaborate and share best practices.
  • Travels up to 80% of the time to facilities and locations within the Minneapolis market

  • Develop relationships with key internal and external stakeholders to collaborate and advance plans to improve on mutually established KPIs
  • Maintain an understanding of current regulatory and market operational challenges that may impact performance
Requirements: We are excited about you if you have these things:
  • Bachelor\xe2\x80\x99s degree in healthcare related field
  • 5 years of strategy, operational or clinical leadership in the post acute industry
  • Demonstrated history of developing and implementing data-based performance improvement strategies
  • An understanding of healthcare economics and regulatory requirements governing skilled nursing/home health, BPCI A, MSSP/ACOs, CMS Waiver Programs, etc.
  • Organizational skills, with an ability to prioritize multiple asks and deadlines
  • Strong leadership and communications skills to achieve buy-in and collaboration toward objectives
  • Experience with Microsoft Office, Word, Powerpoint, Excel, Teams, and other virtual conferencing platforms
