THE POSITION: The Peer Support Specialist will be a member of the Mobile Supportive Services (MSS) Team whose mission is to support those experiencing houselessness transition from shelter to permanent housing and retain housing. The successful applicant will work with shelter teams as well as the Clinical Social Worker on their mobile team to provide peer delivered services to participants in the MSS program, assisting them with skill development, navigating systems of care and community integration. Tasks performed by Peer Support Specialists may include assisting their peers in articulating their goals for recovery, supporting them in their treatment, modeling effective coping techniques and self-help strategies based on the specialist\xe2\x80\x99s own recovery experience, acting as a liaison between them and other providers and supporting them in advocating for themselves to obtain effective services. The position will be a part of a small team who will support and engage in participants overall health and well-being using a strength-based, housing first and harm reductive model.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
  • Develop rapport with participants, utilizing personal and professional experience to empower and promote wellness and recovery
  • Work with shelter staff to conduct pre-enrollment meetings with identified participants, as needed, to build a relationship of trust and rapport and act as a guide into MSS services. Once the participant is in the MSS program, continue to provide regular peer support visits in their home and throughout the community to support, educate and engage participants in their own environment (often by oneself).
  • Work closely with the Clinical Social Worker to advocate, refer, and assist participants with applications to ensure participants get access to a variety of services and benefits, including employment, recovery, and mental and physical health services.
  • Develop comprehensive knowledge of community and housing resources and maintain effective relationships with these resources to best meet participant needs.
  • Assist the Clinical Social Worker with conducting program intakes, exits and follow-ups and ensuring corresponding documentation is accurate and uniform.
  • Maintain accurate physical and electronic records, including utilizing the Homeless Management Information System (HMIS) to record peer services provided.
  • Work with participants to identify and develop natural community supports to foster independence and community integration.
  • Assist participants with tasks related to housing acquisition, including housing search and viewing, submission of applications, lease and compliance paperwork signing and obtaining furniture and household goods.
  • Support participants in advocating for their housing needs and communicating with their property managers. Assist with eviction prevention, including helping participants develop housing stability plans to address identified issues.
  • Work with the MSS Program Manager and Social Worker to prepare participant assistance fund requests.
  • Participate in weekly mobile team case conferencing and other on-going, recurring community partner meetings for both Veterans and non- Veterans.
  • Maintain certification and advance professional knowledge/skills with the guidance of the Program Manager to drive continuous improvement in program effectiveness as measured by individual progress.
  • Maintain effective communication with all staff, programs, and community partners.
  • Other assigned tasks and duties appropriate to position.
  • Adhere to all state and federal privacy and security regulations applicable to the program, and to Do Good Multnomah\xe2\x80\x99s policies and agreements regarding confidentiality, privacy, and security.
