Job Summary:
Promotes quality health care, a safe environment, adequate housing and adequate social services for clinic patients by providing information, education, and assistance.
Job Duties:
Routine duties shall include providing health care services to individuals eligible for services in accordance with the self-governance compact and funding agreement between the Cherokee Nation and United States executed under the authority of the Indian Self-Determination and Education Assistance Act. Makes home visits and performs assessments of patient needs; housing, environment, health and daily activities including transportation. Processes home visit assessments, providing appropriate services to patients. Provides basic home safety and health care information and education to home visit clients, clinic patients, and patients in the community setting. Maintains records and provides reports as required. Provides resource information on health, housing, and social services to patients and assists patient to access services as necessary. Monitors clinic waiting areas for patients that may need assistance. Sometimes patients are overlooked or forgotten. Assists clinic patients in completing applications, survey, forms, etc. Assists clinic patient with resolution of problems or complaints. Assists clinic patients with completion of surveys. Monitors are not always user friendly for all patients. Monitors clinic waiting areas for appropriate magazines, television use, or other distributed information, such as signs. Monitors clinic bulletin boards for accurate and updated information. Attends community events/meetings as an advocate for clinic/patient relations. Assists clinic patients with using the educational computers. Assists in maintaining good public relations. Other duties may be assigned.
