Palliative Care Coordinator
Qualifications:
LPN/LVN (required) Hospice experience 1 year (preferred) Organization Skills, Self-motivated, Assessment knowledge
Job Duties:
Patient Assessment and ability to monitor declines in patient health and disease process Collaboration between patient/family and hospice consultant team for care coordination of patient Ability to follow schedule and make phone calls to patients/family members on a regular basis Clinical charting in the EMR system Knowledgeable of hospice services available to patients/family
