Job Overview

We are looking for a reliable and knowledgeable LPN or RN to join our on-call nursing team! As an on-call nurse, you will be responsible for fielding calls from our 5 Wichita home plusses and delivering immediate nursing interventions, assessing resident needs, and coordinating care during after-hours or urgent situations. Your expertise will ensure seamless resident support and safety around the clock, making a meaningful difference in the lives of our residents. This paid position is ideal for nurses eager to apply their skills flexibly while maintaining a passion for compassionate care. Weekday hours are 5p-7a. Weekend hours are 7a-7a.

Responsibilities

Respond promptly to after-hours calls from our staff or regarding our residents. Perform comprehensive assessments including vital signs, physical examinations, and review of medical histories using ALIS (EHR). Administer medications via injections with strict adherence to aseptic techniques and HIPAA regulations to ensure resident safety. Coordinate with multidisciplinary teams for case management, discharge planning, and follow-up care in the event a resident is discharged back to the homes or transferred out of the homes for evaluation. Maintain effective communication with home health and/or hospice teams as needed. Provide triage services by evaluating symptoms related to infectious disease care, trauma medicine, or behavioral health issues to determine urgency levels. Support residents with complex needs including those in with physical disabilities or memory care residents.

Requirements

Valid RN or LPN license in good standing. Proven experience in long-term care environments such as nursing homes, assisted living communities, or home plus settings or in hospital backgrounds providing geriatric care. Ability to work independently during on-call shifts while maintaining excellent patient assessment skills and attention to detail. Excellent communication skills for effective triage and resident/staff education. Reliable transportation Ability to access EHR remotely
