Job Summary
: The Office/Admin Representative is responsible for coordinating caregiver placement and visit confirmations between the Home Health Aides and our patients. Responsibilities include creating and maintaining staff schedules, confirming attendance and communicating with the caregivers, patients and their families while adhering to Renaissance\'s policies and procedures. He/she performs case management for the assigned caseload and works to provide best quality service.
Job Responsibilities:

  • Assist with application process of new Direct Care Workers to ensure compliance
  • Hold orientation for all new Direct Care Workers
  • Assist with referral management of all new patients and communication with families
  • Provide coordinated care to patients by developing, monitoring, and evaluating their home care care plans
  • Ensure a high level of care for the patient given by the Direct Care Workers
  • Schedule agency Direct Care Workers accordingly and ensuring agency aides work their scheduled shift
  • Manage last minute calls outs, finding appropriate coverage
  • Communicate with referral sources and Case Managers to provide an excellent customer service experience
  • Listen to needs of clients and matching them to the appropriate caregiver(s)
  • Review the care plan with patients and caregiver
  • Resolve caregiver and client grievances and complaints
  • Reporting personnel performance issues
  • Ensure caseload retention

Job Requirements:
  • High School diploma/ GED equivalent
  • 2.5 years of home care experience
  • Knowledge of HHA Exchange is highly preferred
  • Strong interpersonal and communication skills
  • Able to deal with difficult or sensitive situations in person or on the phone
  • Excellent customer service having empathy and patience in working for elderly population
  • Able to multitask and work in an extremely fast paced environment
  • Ability to maintain a professional demeanor when handling issues
