Job Description

Nationwide Healthcare Services is looking for clinicians to join our rehabilitation departments in the Wilmington and Hockessin Delaware areas.



We have Full-Time, Part-Time and PRN positions available.



RESPONSIBILITIES




Therapist Specific




Evaluates residents and develops appropriate care plans.

Monitor residents' progress and adjust treatments accordingly.

Provide supervision for Occupational Therapy/Physical Therapist Assistants, students and support personnel in accordance with state license requirements and professional standards

All Clinicians




Under a physician's order, provides direct resident treatment in accordance with an established plan of care. Treatment modalities may include; individual, concurrent and group.

Provide excellent training for residents and caregivers.

Documents services provided within Rehab Optima on a timely basis.

Demonstrates consistent knowledge and application of Medicare and other reimbursement regulations.

Exhibits courtesy, compassion and respect to residents, families, caregivers, visitors, physicians, administrators, and co-workers in each and every interaction.

Demonstrates continuous new learning to remain current in the field.

Complies with facility Privacy Practices policies and procedures to protect residents' individual health information.

Participates in clinical program development and implementation.

Contributes to the team meeting and decision-making process as defined by the program.

Responsible for service and operational excellence of all assigned activities to ensure the delivery of quality outcomes.

Participates in the training and orientation of other staff, students and volunteers.

Participates in quality improvement activities as needed.

May perform additional assigned administrative duties like: assigning and overseeing resident care activities, medical record oversight, assuring execution of physician orders, ordering supplies, etc.

EDUCATIONAL REQUIREMENTS:




Graduated from an accredited program approved by their professional association.

Current licensure by the applicable state agency.

EXPERIENCE REQUIRED




Clinical experience in a skilled nursing facility is preferred.
