Responsibilities:
Perform comprehensive health assessments, including history and physical examinations.
Diagnose and manage acute and chronic medical conditions.
Provide wound assessments, debridement, dressing changes, and wound care treatment plans.
Develop individualized patient care plans and ensure follow-up care.
Collaborate with physicians, nurses, and other healthcare team members to deliver coordinated care.
Educate patients and families on disease prevention, treatment options, and healthy lifestyle choices.
Maintain accurate and timely documentation in the EMR system.
Ensure compliance with all regulatory and clinical guidelines.
Qualifications:
Current and valid Nurse Practitioner license in [State].
Board Certified (FNP or AGNP preferred).
Minimum 2 years of clinical experience in primary care or wound care (wound care certification is a plus).
Strong clinical assessment and problem-solving skills.
Excellent communication and patient education skills.
Ability to work both independently and as part of a team.
