Job Summary


We are seeking a dedicated and skilled Nurse Practitioners to join our Long-Term Care (LTC) team. The ideal candidate will possess a strong background in geriatrics, acute care, and chronic disease management, with a focus on providing high-quality patient care in nursing home and assisted living settings. You will be responsible for assessing, diagnosing, and managing patient care while collaborating with interdisciplinary teams to enhance the quality of life for our residents.

Responsibilities



Conduct comprehensive physical examinations and health assessments for patients in long-term care settings. Develop and implement individualized care plans based on patient needs and preferences. Manage acute pain, behavioral health issues, and chronic conditions through effective treatment strategies. Collaborate with physicians and other healthcare professionals to coordinate patient care. Maintain accurate and timely medical documentation in compliance with HIPAA regulations using EMR/EHR systems. Provide education to patients and families regarding health management, disease prevention, and wellness strategies. Participate in staff/facility meetings to improve patient outcomes and advance clinical practices. Ensure adherence to infection control protocols and best practices in patient care.

Requirements



Current Nurse Practitioner license in the state of practice with relevant certifications in adult, geriatrics or family practice. Experience in long-term care settings such as nursing homes or assisted living facilities is preferred but not necessary. Proficiency in EMR systems (eClinicalWorks, GEHRIMED, Matrix, PCC ) for effective medical documentation. Knowledge of ICD-9/ICD-10 coding. Background in critical care or hospital medicine is a plus; experience in Level I/II trauma centers is desirable. Strong communication skills with the ability to work collaboratively within a multidisciplinary team environment.
Job Type: Full-time

Pay: $100,589.29 - $200,000.00 per year
