Position Summary
The certified pharmacy technician I - medication reconciliation will perform any/all tasks associated with the medication reconciliation process across the continuum of care. Working interdepartmentally and collaboratively to ensure all patient home medication lists or electronic documentation are accurate and current to provide a reliable source of medication usage to providers
Required Minimum Knowledge, Skills, and Abilities (KSAs)
Education: High school graduate or equivalent preferred.
License/Certifications: Maintains Certified Pharmacy Technician status.
Experience: 2-3 years of hospital pharmacy preferred.
Excellent math skills essential
Computer skills required
