Job Description

Providence Home Health and Hospice is seeing a Licensed Medical Social Worker with experience in the Home Health and Hospice setting. The primary purpose of your job position is to assist in planning, organizing, implementing, evaluating, and directing the overall social service needs of our Home Health and Hospice clients in accordance with current federal, state and local standards, guidelines and regulations, our established policies and procedures and as may be directed by the Administrator, to assure that the medical related emotional and social needs of the resident are met/maintained on an individual basis. You will also help clients understand their diagnosis and make the necessary adjustments to their lifestyle, housing or health care. You will hep arrange for services to help clients by assessing their situation, setting goals and obtaining required services.

EDUCATION

Must possess a Masters Degree in Social Work (MSW) licensed in the State of Kansas in good standing.

EXPERIENCE

Minimum of 1-3 years experience as a licensed social worker preferable in a healthcare/hospice setting. Must be well-organized, an excellent communicator and a proven leader. Fundamental understanding of healthcare/hospice policies.

REQUIREMENTS

Must have a valid driver's license, dependable vehicle and current auto insurance.

Job Type: Full-time

Pay: $62,000.00 - $67,000.00 per year
