Responsibilities

Provide individual and group counseling to patients dealing with mental health issues, chronic illness, or end-of-life concerns.
Collaborate with healthcare providers to ensure coordinated patient care and effective treatment plans.
Facilitate crisis intervention and management for patients experiencing acute distress or emergencies.
Maintain accurate medical documentation and records in compliance with HIPAA regulations.
Engage in case management activities, including referrals to community resources and support services.
Participate in clinical research initiatives to improve patient outcomes and advance social work practices.
Educate patients and families about available resources, treatment options, and coping strategies.
Advocate for patients' rights and needs within the healthcare system.
Provide support in hospice care settings, focusing on comfort and quality of life for terminally ill patients.
Skills
Proficient in child & family counseling, clinical research, and hospital experience.
Strong background in data collection, hospice care, addiction counseling, behavioral therapy, and applied behavior analysis.
Expertise in patient care, patient assessment, medical documentation, medical terminology, and medical records management.
Experience with intake processes, clinical counseling, group therapy, social work practices, behavioral health interventions, and mental health counseling techniques.
Skilled in diagnostic evaluation, case management strategies, individual/group counseling approaches, psychotherapy methods, and motivational interviewing techniques.
Familiarity with assisted living environments and experience working with children is preferred.
Competence in crisis intervention strategies and home & community care experience is essential.
Knowledge of EMR systems is beneficial along with inpatient care experience or nursing home experience.
Proficient in cognitive behavioral therapy techniques as well as working with individuals with developmental disabilities or Alzheimer's care needs.
Experience providing PTSD care or post-acute care services is valuable.
Understanding of CPT coding as well as ICD coding (ICD-9 & ICD-10) is advantageous for documentation purposes.
Background in geriatrics or pediatrics will be considered an asset.
