Shifts Available: Days - 8 hour shifts - FT position




QUALIFICATIONS





To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required.



1. Current RN license to practice in the state of employment.



2. Previous experience in long-term care.



3. Possesses knowledge and understanding of rehabilitative nursing concepts.



4. Clinically competent to utilize the nursing process to perform physical and cognitive assessments of residents.



5. Caring person with a desire to work with the frail and elderly.



6. Displays optimal intra and interdepartmental relationships.



7. Ability to establish and maintain optimum rapport with residents, family members and regulatory agencies.



8. Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation.




ESSENTIAL FUNCTIONS





To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.



a. Perform assessments necessary to accurately complete the minimum data set as well as the Care Area Triggers (CATS) and the Care Area Assessment (CAAS).



b. Complete all assessments within required time frames for newly admitted residents, quarterly reviews. Annual reviews and significant change reviews following RAI Manual Guidelines.



c. Complete all PPS assessments within required time frames for all PPS required assessments, following RAI Manual Guidelines.



d. Complete all entry and/or discharge tracking forms timely, as required per RAI Manual Guidelines.



e. Initiate and Schedule Resident Care Planning and/or Resident Care Conference as needed and following RAI recommendations (unless facility employees Care Plan Coordinator).



f. Make care plan and referral recommendations.



g. Notify Social Services and Dietary Departments when a new MDS is initiated for whatever reason. Clearly define the dates and sections to be completed. Ensure that all IDT members are aware of schedules as it relate to assessments as well as care plan meetings.



h. Schedule weekly Medicare Meeting with IDT members to review all Medicare/Insurance residents as well as review all PPS assessments.



i. Complete all required billing and/or reports as directed and necessary.



j. Utilize direct interview/observation skills, chart audits, resident/family interviews and communicate with direct care staff when completing the assessments.



k. Follow all facility policies related to the maintenance of an accurate medical record.



l. Maintain a clean, safe and efficient work environment.



m. Use time, equipment and supplies in a responsible manner.



n. Attend nurses' meetings as scheduled.



o. Attends required in-services and completes assigned on-line modules.



p. Meet state and federal continuing education requirements.
