Job Description

Heritage Pointe of Fort Wayne is seeking an MDS Coordinator to oversee the completion and submission of Minimum Data Set (MDS) assessments and care plans for our residents. This key role ensures accuracy, timeliness, and compliance while supporting our commitment to quality care.


Essential Job Functions




Assessment Management:




Coordinate and conduct resident assessments in accordance with CMS guidelines (MDS 3.0). Ensure accurate and timely completion and submission of MDS assessments. Collect and analyze data to determine resident care needs.

Care Planning:




Collaborate with interdisciplinary team members to develop individualized care plans based on MDS data. Monitor and update care plans regularly to reflect changes in resident conditions.

Regulatory Compliance:




Ensure compliance with federal and state regulations related to MDS and care planning. Maintain records for audits and inspections by CMS and state agencies.

Reimbursement and Documentation:




Accurately code MDS to reflect services provided and support Medicare/Medicaid reimbursement. Stay updated with RAI (Resident Assessment Instrument) guidelines and reimbursement regulations.

Team Collaboration:




Educate and support nursing staff in documentation and assessment practices. Participate in quality improvement initiatives.



Job Qualifications




Education:




Registered Nurse (RN) license required; LPN/LVN may be accepted in some settings.

Experience:




1-2 years of experience in long-term care or MDS coordination preferred. Working knowledge of MDS 3.0, RAI process, and PPS (Prospective Payment System).

Skills:




Strong analytical and organizational skills. Proficient in electronic health record systems. Excellent communication and teamwork abilities. ICD-10 coding skills
