We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. Duties
Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation.
Administer medications and injections accurately following physician orders while adhering to safety protocols such as sterile technique and aseptic procedures.
Assist with medical procedures such as catheterization, wound suturing, phlebotomy, and specimen collection/processing.
Monitor patients' conditions closely, including respiratory status with ventilator management, cardiac monitoring, and pain assessment for acute or chronic conditions.
Provide specialized care for patients with complex needs such as dementia, Alzheimer's disease, memory care, or behavioral health challenges.
Support emergency situations by participating in airway management, IV infusion setup, and responding to urgent care or trauma cases within Level I or Level II trauma center environments.
Maintain accurate medical documentation within Electronic Health Record (EHR) systems like Epic or Cerner while complying with HIPAA regulations.
Educate patients and their families on health coaching, discharge planning, medication management, and post-acute care instructions.
Collaborate with interdisciplinary teams including physicians, case managers, physical therapists, and social workers to coordinate comprehensive patient care plans.
Ensure infection control standards are strictly followed through proper sterilization techniques and aseptic methods during procedures such as sterile processing or dialysis.
Assist in medical imaging procedures like sonography or cardiac catheterization when necessary.
Support patients across various settings including outpatient clinics, inpatient units such as ICU or pediatric units like PICU, home & community care environments, assisted living facilities, nursing homes, and hospice care programs.
Skills
Proven experience working with Electronic Medical Records (EMR) systems such as Epic, Cerner, Athenahealth, or eClinicalWorks.
Strong clinical skills in areas such as IV insertion/infusion therapy, medication administration (including injections), tube feeding management, and catheter care.
Extensive knowledge of physiology, anatomy, medical terminology, ICD coding (ICD-9/ICD-10), CPT coding for procedures like suturing or gastric lavage.
Critical care experience including ICU or emergency medicine environments; familiarity with Level I/II trauma centers is highly desirable.
Experience providing specialized dementia care including Alzheimer's disease management and memory support strategies.
Skilled in patient assessment techniques for diverse populations including pediatrics (neonatal ventilator management), adults with acute pain management needs or infectious diseases.
Ability to perform diagnostic evaluations such as laboratory testing (phlebotomy), specimen processing, or radiologic procedures like sonography.
Knowledge of infection control practices to prevent healthcare-associated infections across all settings.
Competence in handling complex cases involving hospice care coordination or post-acute sub-acute rehabilitation services.
Excellent communication skills for effective collaboration within multidisciplinary teams and with patients' families.
