The

Housing Case Manager

provides on-site coverage and case management and counseling services to residents of our residential program. The role focuses on helping participants secure stable housing, develop essential life skills and achieve greater self-sufficiency. The ideal candidate is a strong advocate who consistently puts clients' needs first and embodies our core values.




Key Responsibilities:




Housing Assessment and Placement




Conduct comprehensive housing assessments to identify each participant's strengths, resources, and barriers. Guide clients through the entire application process, gathering required documentation and completing forms. Assist with move-in logistics, unit inspections, and the creation of individualized rent-payment and crisis-management plans. Identify and prepare participants for transitional housing opportunities.

Post-Placement and Case Management Support




Provide ongoing case management after housing placement, helping clients adjust to independent living Facilitate the transition to new homes, addressing issues such as bill payment, visitor management, and social isolation Connect clients with community services. Coordinate transportation to housing fairs, appointments, and related events. Provide walk-in service and crisis-line support. Co-facilitate shelter support groups.

Landlord & Agency and Community Relations




Recruit and maintain relationships with landlords willing to accept subsidized housing clients. Liaise with the housing authority to secure leases and ensure long-term housing stability. Represent the organization and act as a liaison for staff and community partners on designated topics. Connect participants with mental-health, physical-health, substance-recovery, and other supportive services based on intake assessments. Contribute to special projects such as the Holiday Bazaar and other community-education initiatives.

Documentation, Reporting, and Program Collaboration




Keep accurate, up-to-date client records in compliance with agency policies, funding requirements, and legal standards. Compile data, generate reports, and maintain concise case notes. Assist the Director of Shelter Programming with Intake interviews as needed. Participate in regular supervision, program meetings, and volunteer training sessions.

Basic Computer Proficiency:

Experience with Microsoft Word, Excel, PowerPoint, Outlook, and similar tools.

Education:

Bachelor's or master's degree in social work, Psychology, Human Services, or a related field.

Experience:

+

3+ years

of supervisory/direct-service, crisis-intervention, and advocacy work,

OR

+

5+ years

of comparable experience in the field.

Preferred Experience:

Prior work with victims of domestic and/or sexual violence and trauma informed care is strongly preferred.

Other:

Successful completion of a background check meeting InterAct's standards and adherence to InterAct's Communicable Disease protocols.
