The Beth-El Center is a 501(c)(3) organization that provides support services and advocacy to those experiencing homelessness and hunger within the Greater Milford community. Through our 34-bed homeless shelter, permanent supportive housing, food service programs, diversion services, and outreach and engagement efforts, we offer individuals, families, and veterans' long-term solutions to help them reclaim their lives.

SUMMARY

Under the general direction of the Executive Director, the Case Manager/SOAR Specialist provides case management, support services, and housing stabilization assistance to residents in the supportive housing property at 85 New Haven Avenue, Milford, and SOAR services to qualified members of the Beth-El Center community. The Case Manager/SOAR Specialist ensures resident safety, promotes independence, and strengthens connections to community resources while ensuring compliance with state and federal requirements.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Permanent Supportive Housing Case Management

Identify residents in need of services and monitor overall health status, including medical, mental health, and/or substance use issues. Conduct regular case management meetings, maintain service documentation, and develop individualized service plans (updated every 6 months). Provide assessments, referrals, crisis intervention, conflict resolution, and low-intensity counseling. Serve as a liaison between property management, tenants, and service providers. Promote client self-sufficiency and independence while complying with all protective services reporting procedures. Organize community-building activities; attend monthly community meetings with tenants and property management. Maintain accurate records in compliance with Department of Mental Health and Addiction Services (DMHAS) and HMIS reporting requirements. Oversee DMHAS site monitoring, including audits, inspections, tenant interviews, and compliance reports. Provide transportation support for residents' medical, mental health, and basic needs appointments. Develop monthly program calendar with skill-building opportunities such as money management, employment services, and peer support. Provide 24-hour on-call support to residents.
EDUCATION AND EXPERIENCE

Bachelor's degree in social work, human services, or related field (may substitute one year of experience). SOAR Certification, Preferred At least two years' experience working in human services. Strong interpersonal, verbal, and written communication skills; ability to work effectively with a diverse client and funding base. Demonstrated ability to build empathetic, supportive relationships with individuals experiencing homelessness. Familiarity with community resources, benefits programs, and supportive housing preferred.
