Location: Omaha Market/Lincoln Market Azria Health is a skilled nursing organization with 20 facilities located across Nebraska, Iowa and Kansas. We are currently looking for a Hospital Liaison to join our team! This Liaison will specify in the Omaha, Lincoln, and Council Bluffs areas. Successful candidates will be highly professional, comfortable with patients and families and able to communicate well with nurses and case managers in the referring hospital networks. We are looking for a highly motivated individual to fill this position. A Hospital Liaison on our team will follow the direction of the Healthcare Center administrators and be accountable for forming and maintaining positive relationships with potential patients and key members of our referring hospital networks. Candidates Will Have

  • Must possess a comprehensive knowledge of Skilled Nursing and long term care, as normally obtained through a Bachelors degree in nursing or a related program or experience as a Liaison, LTC nurse or case manager. A combination of education and experience may be acceptable.
  • Must be comfortable speaking/negotiating with discharge planners and insurance case managers.
  • Understanding of various insurance methodologies, including Medicare, Medicaid, and Managed Care insurance
  • Strong understanding and application of the quality process and tools
  • Candidates should have excellent communication and presentation skills, with the ability to communicate with multiple levels in the organization.
  • Candidates should also possess the ability to work as part of a team, coach and train staff, as well as strong analytical and problem-solving skills.
  • Must demonstrate analytical ability and possess a comprehensive knowledge of basic data analysis and communication tools including, Excel, Access and Word.
  • Must be comfortable operating in a collaborative, shared leadership environment.
