Job Title: RN On-Call for Hospice
Job Summary: As an RN On-Call for Hospice, you will provide compassionate and skilled nursing care to terminally ill patients and their families during after-hours and weekends. You will be responsible for assessing patients' needs, managing symptoms, providing emotional support, and coordinating with interdisciplinary teams to ensure the delivery of high-quality end-of-life care. Your expertise and availability during non-business hours will contribute to maintaining a continuous and responsive hospice care service.
Responsibilities:
Provide skilled nursing care to hospice patients in their homes, nursing facilities, or hospice residences during on-call shifts.
Conduct thorough assessments of patients' physical, emotional, and psychosocial needs, including pain and symptom management.
Develop and implement individualized care plans in collaboration with the interdisciplinary team, patient, and family, focusing on patient comfort and quality of life.
Administer medications, treatments, and procedures as prescribed by physicians and hospice protocols.
Monitor and evaluate patients' condition, promptly reporting any significant changes to the hospice team and initiating appropriate interventions.
Educate patients and their families about the disease process, end-of-life care options, and available community resources.
Provide emotional support, counseling, and guidance to patients and families, addressing their fears, concerns, and coping mechanisms.
Coordinate care with other healthcare professionals, including physicians, social workers, counselors, and volunteers, to ensure holistic care delivery.
Document all assessments, interventions, and communication accurately and promptly in the electronic medical record system.
Maintain confidentiality and adhere to legal and ethical standards in accordance with hospice policies and regulations.
Participate in on-call rotation schedules, being available during non-business hours, including evenings, nights, weekends, and holidays.
Demonstrate commitment to professional growth by engaging in continuing education, workshops, and other learning opportunities.
Qualifications:
Valid and unrestricted Registered Nurse (RN) license in the state of practice.
Minimum of one year of nursing experience,.
Strong knowledge and experience in pain and symptom management for terminally ill patients.
Excellent assessment, critical thinking, and problem-solving skills.
Effective communication and interpersonal skills to interact with patients, families, and interdisciplinary team members.
Compassionate and empathetic approach towards patients and families during the end-of-life journey.
Ability to work independently, make sound judgments, and prioritize tasks during on-call shifts.
Proficient in using electronic medical record systems and other relevant healthcare software.
Understanding of the principles of confidentiality and HIPAA regulations.
Willingness to participate in an on-call rotation, including evenings, nights, weekends, and holidays.
Job Types: Full-time, Part-time, Per diem
Pay: $55,000.00 - $60,000.00 per year
