Job Overview

We are seeking a dedicated and skilled Registered Nurse Case Manager to join our healthcare team. The ideal candidate will have a strong background in nursing, particularly in hospital settings, and possess the ability to manage patient care effectively. This role involves coordinating patient services, ensuring quality care, and collaborating with multidisciplinary teams to optimize patient outcomes. The Registered Nurse Case Manager will work primarily within a Level II trauma center environment, focusing on inpatient care and emergency room services.

Responsibilities

Assess and evaluate patient needs through comprehensive medical records review and physical examinations. Develop individualized care plans based on thorough knowledge of physiology and critical care protocols. Monitor vital signs and respond promptly to any changes in patient conditions. Collaborate with physicians, nurses, and other healthcare professionals to ensure seamless patient service delivery. Facilitate communication between patients, families, and healthcare teams to enhance understanding of treatment plans. Advocate for patients' needs and preferences throughout their hospital stay. Educate patients and families about health management strategies post-discharge. Maintain accurate documentation of patient interactions, care plans, and progress notes.

Experience

Current Registered Nurse (RN) license required. Previous experience in a hospital setting is essential, with a focus on critical care or emergency room environments preferred. Familiarity with medical records management and inpatient care processes is necessary. Strong knowledge of physiology related to patient assessment and intervention techniques. Excellent interpersonal skills for effective communication with patients, families, and healthcare teams. Ability to work in a fast-paced environment while managing multiple priorities efficiently.

Job Type: Part-time

Pay: $28.00 - $40.00 per hour

Schedule:

Monday to Friday On call Weekends as needed
Work Location: On the road
