-Previous RN Hospice Case Manager Experience a MUST

We are a small hospice organization that services the West/East valley areas. We are a strong team of professionals that enjoy working with Hospice patients and their families during end of life scenarios. Seeking an experienced RN for Hospice Case Management, admissions and on-call visits. Must be flexible and available for needs of patients. You will be responsible for case management, interfacing with core hospice staff, supervising hospice aides, patient record keeping on EHR and attending mandatory bi-weekly IDG meetings. Must have a registered nursing degree, valid auto insurance, proof of current TB test and if received, Covid Vaccination record.
