Job Description
Hospice Care Team is looking to hire a full-time Registered Nurse Case Manager to assist with our hospice patient visits in the Pearland area! The RNCM will work with a team of healthcare providers to ensure our home health patients are receiving quality care.
Responsibilities:
Conducting routine visits, admissions visits and/or follow-up visits at home or other facilities settings. Coordinating, assessing, and implementing patients' care. Providing accurate and timely documentation of patient services to reflect plans of care. Flexibility to be available days, evenings and weekends.
