Registered Nurse (RN) Hospice Case Manager

to join our growing home hospice team, supporting patients and families throughout the

Metro Atlanta Area

.

In this vital role, you will be the cornerstone of our patients' care in the comfort of their own homes. You will provide holistic, patient-centered care, collaborating with an interdisciplinary team to ensure physical, emotional, and spiritual well-being for patients facing life-limiting illnesses.

Key Responsibilities:



Comprehensive Patient Assessment:

Conduct thorough initial and ongoing assessments of patient needs, including physical, psychosocial, and spiritual aspects.

Individualized Care Planning:

Develop, implement, and evaluate individualized care plans in collaboration with the patient, family, physician, and interdisciplinary team (IDT) members (social worker, spiritual counselor, hospice aide, volunteer).

Symptom Management:

Provide expert symptom management, pain control, and comfort care, educating patients and families on medication administration and non-pharmacological interventions.

Care Coordination:

Act as the primary point of contact for patients and families, coordinating all aspects of care, including scheduling visits, ordering supplies, and arranging for necessary services.

Patient and Family Education:

Educate patients and families on disease progression, end-of-life care options, self-care techniques, and emotional support resources.

Interdisciplinary Team Collaboration:

Actively participate in IDT meetings, presenting patient updates, collaborating on care strategies, and ensuring seamless communication among team members.

Documentation:

Maintain accurate, timely, and thorough electronic medical records in compliance with all regulatory standards and hospice policies.

On-Call Rotation:

Participate in a rotating on-call schedule to provide support and address urgent patient needs outside of regular business hours.

Adherence to Regulations:

Ensure compliance with all federal, state, and local regulations, as well as hospice policies and procedures.

Qualifications:



Current and unrestricted

Registered Nurse (RN) license

in the state of Georgia. Experience as a Registered Nurse,

2 year of experience in hospice, palliative care, or home health highly preferred.

Current BLS certification. Strong clinical assessment and critical thinking skills. Demonstrated ability to provide compassionate, empathetic, and patient-centered care. Excellent communication, interpersonal, and organizational skills. Proficiency in electronic medical records (EMR) systems. Ability to work independently and as part of a collaborative team. Reliable transportation, valid driver's license, and automobile insurance. Comfortable providing care in diverse home environments within the Metro Atlanta area.

Pay: $70,613.45 - $93,340.93 per year
