Hospice Case Manager

Job Overview

We are seeking a compassionate and experienced Hospice Case Manager to coordinate and deliver comprehensive end-of-life care for patients in various settings. The ideal candidate will possess an expertise in patient assessment, discharge planning, and interdisciplinary collaboration. Duties

Conduct thorough patient assessments to determine care needs, including vital signs, physiological status, and psychosocial factors. Develop and implement individualized care plans in collaboration with healthcare teams, patients, and families. Coordinate services across inpatient, outpatient, clinic, and post-acute settings to ensure seamless transitions of care. Manage discharge planning processes, including documentation and communication with relevant providers and caregivers. Monitor patient progress through diagnostic evaluations, medical records review, and ongoing assessments. Facilitate utilization management by evaluating the appropriateness of services and resource utilization. Maintain compliance with HIPAA regulations to protect patient confidentiality and privacy. Collaborate with multidisciplinary teams including nursing, social work, physicians, and therapists to optimize patient outcomes. Educate patients and families on disease processes, symptom management, and available resources related to hospice care. Document all interactions accurately within medical records systems and ensure timely updates.

Requirements

Graduate of an accredited school of medicine, social work, nursing or related medical occupation. At least 3 years of Case management experience. Previous experience with coordinating community resources for post-acute hospitalization care preferred. Active Oregon RN License
