Care Coordinator



JOB SUMMARY:



The Care Coordinator is responsible for intakes, clients, caregivers, and scheduling. This individual directly interfaces with clients performing service evaluations, originating service plans, scheduling and supervising care.

QUALIFICATIONS:



1. H.S. Diploma or GED, some college or two years related experience and/or training preferred.
2. Experience in home care or knowledge of the health care industry preferred.
3. Previous management experience.
4. Prior scheduling experience preferred.
5. Requires proficiency in computer skills including but not limited to MS Office, MS Excel and Scheduling program.
6. Have a sympathetic attitude toward the care of the sick and elderly.
7. Ability to carry out directions, read and write.
8. Maturity and ability to deal effectively with the demands of the job.
9. Must possess and demonstrate excellent communication, leadership and organizational skills and customer service techniques.
10. Must perform and manage multiple responsibilities concurrently and work well under pressure.

RESPONSIBILITIES:



Answers the telephone and performs Intakes. Conduct care consultations with prospective clients and families Manage the client intake process Facilitate client/caregiver introductions with every new client and with every new caregiver Oversee the hiring process for caregivers and manage orientation/onboarding Work closely with HR to provide support caregivers, coaching and ongoing caregiver training. Schedules shifts by matching caregiver qualifications and availability to client's needs. Supervises caregivers Understands and adheres to established FirstLight Home Care policies and procedures. Handles coordination of care for client services. Reports changes in client condition to agency employees involved in client's care. Takes calls after hours and weekends to handle emergencies, scheduling issues, and client referrals and intakes. May perform continuing and on-going caregiver training. Participate in the quality assurance reviews and evaluations of the agency's services. Conduct care consultations with prospective clients and families
WORKING ENVIRONMENT:

Works indoors in FirstLight Home Care office and client homes and travels to/from client homes.

Pay: $17.00 - $19.00 per hour
