Salary offers are based on the candidate\'s equivalent experience and internal equity with other employees within the same job classification. About the Position The HIV Jump Start Specialist will be responsible for working with people living with HIV who are disengaged from HIV primary care following their release from recent jail incarceration. The Jump Start Specialist navigates health and social services to ensure clients are connected to HIV health care and HIV medical case management.

We Require
Bachelor\'s degree in social work, health promotion, public health, medical technology, health sciences or another closely related field.
Two years of paid experience working in case management, social services or advocacy role
Valid Arizona driver\xe2\x80\x99s license upon hire
We Also Value
Experience working with persons living with HIV conducting health assessments and field investigations, conducting community outreach, performing case management duties, and/or counseling clients
Bilingual (English/Spanish) skills
Job Contributions
Coordinate with Correctional Health Services of Maricopa County and the Ryan White Part A HIV Nursing staff to identify patients who have fallen out of care, have never been in HIV care, or who are at risk of falling out of care based on eligibility assessment and linkage to care status
Conduct eligibility assessments and goal plan development with the patient to reduce barriers to retention and adherence
Conduct client field and telephone investigations with utmost regard to confidentiality by researching, identifying, locating, and contacting people in Maricopa County who are co-infected with HIV and other sexually transmitted infections and refer those clients to a medical provider for diagnosis and treatment
Interview patients who have HIV to elicit contacts and obtain epidemiological information, such as risk factors and previous disease status
Create and maintain case investigation documents, records, and reports
Conduct community field visits and outreach activities targeting high-risk communities, which involve registering clients for testing, performing venipuncture, and obtaining specimens
Provide assistance for Linkage to Care to clients who present to MCPHD clinic who were recently released from jail or state prison
Working Conditions
Ability to participate in emergency response training as scheduled and be available to respond as directed in the event of a public health emergency
Requires travel to and from job-related locations during the course of a scheduled workday, subject to County policies regarding the use of County vehicles and/or private vehicles used on County business
All employees, within one month of hire, will be required to provide evidence of immunity or receive immunizations for vaccine-preventable illnesses for any position within the Maricopa County Department of Public Health. Required immunizations include MMR (measles, mumps, rubella), Tdap (whooping cough and diphtheria), Varicella (chicken pox) and annual Influenza and some positions require a Hepatitis B vaccine and/or a Tuberculosis test
