The HIM clerk is responsible for working with patient and other requestors, including, but not limited to insurance representatives, attorneys, researchers for paper and electronic health record access. This may include scheduling an appointment for viewing or processing the release to ensure minimum necessary regulations are applied. This position is responsible for ensuring all aspects of release of information (ROI) are completed accurately and within required timeframes.
The HIM clerk is also responsible for working with medical staff providers and other hospital colleagues to ensure health records are complete, meeting all required state, federal and CLH policies/regulations. The colleague will work closely with the external coding vendor to coordinate completion of needed documentation for coding completion. Timely analysis of the health record and notification to medical staff providers shall be handled promptly, efficiently and courteously.
The colleague requires a considerable amount of knowledge of the documentation present in the health record, as well as adherence to state and federal regulations and CLH policies pertaining to privacy and security of Protected Health Information (PHI), including ROI processes.
The colleague will comply with departmental and facility policy and procedures and is held accountable to standard of conduct outlined in policies. The position will recognize potential safety hazards to maintain a culture of safety on a continual basis.
