Geriatric Support Services Coordinator, social services - FT/PT - hybrid schedule \xe2\x80\x93 Hiring Bonus!

Dynamic social service agency seeks a Geriatric Support Services Coordinator (GSSC) to work as a member of the team with NaviCare to identify and monitor clients for an all-inclusive care program. GSSC will conduct initial assessments, complete home visits and meet regularly with an interdisciplinary team to develop and monitor care plan. Bilingual applicants encouraged to apply.

Applicant must be organized, possess good writing and communication skills, and have ability to work independently and as a member of an interdisciplinary team. Training provided. Bachelor\'s Degree and two years of professional experience preferred in a related social service field. Bilingual skills and/or 3 years of experience in a related field may be substituted for education. Case management or experience with elders preferred. Excellent communication, written, and computer skills necessary. Use of a reliable vehicle including active vehicle insurance with a current license to drive required.

• A hiring bonus will be awarded in 2 parts, half at 120 Days, and half at 1 year. The bonus for this position is $1,000 (full-time).
