Uniontown Healthcare and Rehabilitation Center is currently seeking a Social Services Coordinator 3 days a week dayshift with a few rotating weekend days. What you get from us as a Social Services Coordinator

What will you do as a Social Services Coordinator

• Assist with coordinating any resident changes such as room transfers, cable, pharmacy, etc; and notifying family members as needed.
• Coordinate staff training sessions such as Residents\' Rights, Guest Relations and Resident Dignity.
• Assist in coordinating Discharge Planning Service (placement options, discharge times, special needs etc) and initiate follow up calls within 48 hours.

What you need as a Social Services Coordinator

• Previous experience in a long-term care facility
• Degree in Social Work or related field
