The Southcentral Foundation (SCF) Elder Resource Specialist is responsible for partnering with assigned Elders and caregivers to provide information, assistance, and referral to community resources, transportation to medical appointments, essential services, and in-home or group visit services as defined in their service plan with the goal of keeping Elders living independently for as long as possible.

The Elder Resource Specialist demonstrates the ability to collaborate with customer-owners, clinical staff within the SCF Primary Care Center and Alaska Native Medical Center Campus and other local medical facilities, municipal, state, and federal agencies and private legal businesses within the community regarding the synchronization of a broad range of services and resources as well as time-intensive care coordination tasks.

This position has two (2) levels designed to provide progressively more responsible and independent work experiences. Progression between job levels is based on the demonstrated ability to successfully handle more progressively responsible assignments.

Qualifications:

SCF programs are established to serve a primary population comprised of Alaska Native people who are affiliated with Cook Inlet Region, Inc. (CIRI) and Alaska Native and American Indian people within SCF\xe2\x80\x99s geographical service area. Employees should have a thorough understanding of the cultures and the needs of this population. Such knowledge is critical to ensure the achievement of SCF\xe2\x80\x99s vision of a Native Community that enjoys physical, mental, emotional, and spiritual wellness, and mission of working together with the Native Community to achieve wellness through health and related services: * Associates degree in Health Care, Human or Social Services, or closely related field from an accredited college or university; OR equivalent combination of education, training, and experience.

  • First Aid/CPR certification or must acquire within six (6) months of date of hire.
  • Must meet the following conditions to qualify as an approved SCF driver:
  • Valid Alaska driver\xe2\x80\x99s license that meets State of Alaska Division of Motor.
  • Vehicles (DMV) license requirements to include:
  • May drive in Alaska for up to 90 days with a valid out-of-State license, then must obtain an Alaska driver\xe2\x80\x99s license.
  • For Commercial driver licenses, you must obtain an Alaska CDL within 30 days of becoming a resident
  • No more than three (3) moving traffic violations (as defined by Alaska Statutes Title 28); OR two (2) at-fault traffic accidents in the previous three (3) years.
  • The driver must not have had \xe2\x80\x98Driving Under the Influence\xe2\x80\x99 (DUI); OR \xe2\x80\x98Driving While Intoxicated\xe2\x80\x99 (DWI); OR \xe2\x80\x98Operating Under the Influence\xe2\x80\x99 (OUI) violations as follows:
  • Zero (0) violations in the past five (5) years.
  • No more than two (2) violations in the past ten (10) years.
  • Drivers must be at least 18 years of age. Drivers 18 through 20 years of age may only transport equipment, materials, and employees. Drivers who transport customers must be 21 years of age or older and must have completed the probationary period of license requirements for Alaska.
  • Prior to hire date, must provide current copy no more than 60 days old of driving record from issuing State.
Additional Qualifications for Elder Resource Specialist II: * Bachelor\xe2\x80\x99s degree in Health Care, Human or Social Services, or closely related field; OR equivalent combination of education, training, and experience; OR demonstrated proficiency as an Elder Resource Specialist I.

Working Environment:
  • Individuals may be exposed to: bloodborne pathogens (over 2/3 time); fumes or airborne particles (under 1/3 time); hazardous waste (under 1/3 time); outdoor weather conditions (under 1/3 time).
  • Noise Level: moderate noise (i.e. business office with computers and printers; light traffic).
  • Local Travel: SCF serves communities throughout the State of Alaska. Many of the communities are accessible via the Alaska road system. Individuals in this position can be required to travel to road-accessible facilities by either driving or being a passenger in a vehicle or by use of alternative transportation.
  • Remote Travel: SCF serves communities throughout the State of Alaska. Many of the communities are rural and cannot be accessed via the Alaska road or rail system. Individuals in this position can be required to travel to rural locations via various modes of transportation which can include small airplanes with additional local transportation by boat, snow machine and/or all-terrain vehicle.
Physical Demands:
  • Position Requirements: exposure to fumes or airborne particles (under 1/3 time); reach with hands and arms (to 2/3 time); sit (under 1/3 time); stand (to 2/3 time); stoop, kneel, crouch, or crawl (under 1/3 time); talk or hear (over 2/3 time); use hands for dexterity to handle or feel (over 2/3 time); walk (under 1/3 time).
  • Weight Lifted or Force Exerted: up to 10 pounds (over 2/3 time); up to 25 pounds (under 1/3 time); up to 50 pounds (under 1/3 time).
  • Special Vision Requirements: close vision (clear vision at 20 inches or less); distance vision (clear vision at 20 feet or more); color vision (ability to identify and distinguish colors); peripheral vision (ability to observe an area that can be seen up and down or to the left and right while eyes are fixed on a given point);depth perception (three-dimensional vision; ability to judge distances and spatial relationships); ability to adjust focus (ability to adjust the eye to bring an object into sharp focus).
  • Additional Physical Demands: Individuals in this position are required to drive SCF vehicles; individuals in this position must be able to respond when needed by prompt appearance at a required location; individuals in this position can be required to provide customer-owner care in the customer-owner\xe2\x80\x99s home or in other locations away from primary worksite; individuals in this position are able to work under physically and emotionally-stressed conditions.

Under P.L. 93-638, as amended, the company pursues a policy of Native preference in hiring, contracting and training. SCF Human Resources must receive certification before applicants receive preference.

Employee Health Requirements:

Compliance with our Employee Health Procedure is a condition of SCF employment. You are required to agree that you will comply with all job-related employee health screening and immunizations prior to your first day of employment. Jobs designated as a Health Care Personnel (HCP) position, requires that you have documentation that you have completed the following immunizations prior to your first day of employment: MMR (Measles, Mumps and Rubella, Varicella (Chicken Pox), Hepatitis B, Influenza, T-dap (Tetanus - Diphtheria - Pertussis), and COVID-19 vaccination is required.
