Provo Canyon Behavioral Hospital (PCBH) is an 80-bed facility, serving the Adolescent, Adult, and Detox/Rehab population and is located in Orem, Utah.

PCBH is seeking an organized Discharge Planner. In this position, you will be responsible for coordinating aftercare and providing support to patients throughout their discharge process.

Qualifications:

• Bachelor\xe2\x80\x99s Degree required in a related field is preferred
• Clerical and organizational experience is required
• Excellent written and oral communication skills, time management, and social skills

Discharge Planner Details:

• This is a Full-time position Monday \xe2\x86\x92 Friday

• Day Shift
