Summary:

The Director of Population Health Management will lead existing clinical programs in diabetes, heart failure, COPD, and hypertension, as well as the pharmacy program which includes the prescription refill and prior authorization program and the pharmacy residency program. This director will be responsible for designing and leading the implementation of population health management programs and pharmacy programs at Coastal with the goals of improved clinical outcomes, patient satisfaction, clinician well-being and lower costs of care.

Responsibilities:

\xc2\xb7 Lead and manage existing clinical programs including COPD, Diabetes, CHF, and hypertension.

\xc2\xb7 Oversees and facilitates Care Conferences for clinical programs.

\xc2\xb7 Leads the development and implementation of patient-focused and coordinated delivery of pharmacy services at Coastal Medical Lifespan.

\xc2\xb7 Collaborates with population health management, data analytics, and the management team to identify and address the clinical needs of Coastal patients.

\xc2\xb7 Tracks key performance indicators for pharmacy and population health management programs and reviews and analyzes data to monitor program performance.

\xc2\xb7 Collaborates and consults with peers and colleagues to define standards and translate them into usable policies, procedures, and standards of clinical care that improve the quality of care and the patient experience of care.

\xc2\xb7 Designs and implements programs to favorably impact pharmacy and hospital utilization and identify and recommend opportunities to reduce cost.

\xc2\xb7 Continually reviews and analyzes pharmacy and population health management operations, recommending and implementing operational changes, allocation of resources, and new technologies or ideas that will increase the efficiency and level of service of the clinical programs.

\xc2\xb7 Ensures clinical quality standards are upheld and contractual reporting deliverables are met.

\xc2\xb7 Develops and implements patient and provider education materials and tools for clinical initiatives.

\xc2\xb7 Collaborates with appropriate members of Coastal\xe2\x80\x99s management team to understand available utilization data and work with the CMO to develop plans to improve clinical processes.

\xc2\xb7 Collaborates with PCPs, patients, and members of the healthcare team, including required health care services outside of Coastal Medical.

\xc2\xb7 Identify and utilize cultural and community resources to address the needs of identified patients and populations.

\xc2\xb7 Takes responsibility for the recruitment, retention, performance enhancement, and competency assessment of subordinate personnel in collaboration with other managers and with the input of direct report supervisors and key constituents.

\xc2\xb7 Ensures that Coastal complies with all clinical pharmacy regulations.

\xc2\xb7 Works effectively with external stakeholders (e.g., payers, government agencies, community partners) as needed to share data, collaborate around new clinical initiatives, or otherwise represent the interests of the organization.

\xc2\xb7 Serves on the Adult and Pediatric Clinical Practices Committees, the Lifespan Pharmacy & Therapeutics Committee, and the Lifespan Ambulatory Pharmacy & Therapeutics Committee.

\xc2\xb7 Participates in or leads additional committees, workgroups, and performance improvement teams as needed.

\xc2\xb7 Meets with the CMO weekly.

\xc2\xb7 Performs other job-related duties as assigned.

Other information:

Education and Experience

BS in nursing, health administration, pharmacy or another clinical field. RN and master\xe2\x80\x99s in health care management preferred.

\xc2\xb7 Current Rhode Island professional license in good standing

\xc2\xb7 Minimum five years of experience in a clinical setting including three years of management experience. Primary care experience preferred.

Knowledge, Skills and Ability

\xc2\xb7 Knowledge of evidence-based Clinical Practice Guidelines.

\xc2\xb7 Knowledge of quality improvement models, processes and tools and their use in a health care setting. Current knowledge of regulatory trends

\xc2\xb7 Knowledge of how to use Electronic Medical Record and computerized statistical methods in clinical quality improvement context.

\xc2\xb7 Strong initiative, goal orientation and follow through, assuring fulfillment of plans

\xc2\xb7 Strong assessment, critical thinking, judgment, creativity and decision-making skills

\xc2\xb7 Effectively provides instruction, coaching and mentoring

\xc2\xb7 Excellent communication skills; both verbal and written

\xc2\xb7 Skill in effective application of Evidence-Based Clinical Practice Guidelines.

\xc2\xb7 Skill in effective application of quality principals, quality measures, and process management initiatives.

\xc2\xb7 Ability to work effectively in a complex environment by establishing effective working relationships with all departments and levels of staff.

\xc2\xb7 Ability to communicate Clinical Quality and Disease Management concepts, methods and tools in a clear and persuasive manner in verbal and written forms to a variety of audiences.

\xc2\xb7 Ability to lead and manage teams of clinical and non-clinical professionals.

\xc2\xb7 Ability to adapt to a fast-paced environment
