The position is part of the Self-Direction Program, and we work with many different FI agencies.

Responsibilities



Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support, using equipment such as Hoyer lifts when necessary. Prepare nutritious meals in accordance with dietary guidelines and assist residents during mealtimes to promote independence. Maintain a clean, safe environment by performing routine cleaning tasks and adhering to infection control protocols. Document care activities accurately in compliance with privacy laws and organizational policies, including progress notes and incident reports Encourage social interaction and engagement in community activities and support participation in preferred activities such as music, movies, pet interaction, and simple cooking activities (with hand-over-hand assistance as needed) Maintain close supervision at all times to ensure safety as required. Skills



Proven experience working in group homes, assisted living, nursing homes, or home health environments with individuals who have developmental disabilities or aging-related conditions is a plus Familiarity with emergency procedures such as CPR and first aid certification to respond effectively to crises is a plus. Strong communication skills for collaborating with healthcare teams, residents, families, and community resources Ability to perform physical tasks including heavy lifting and assisting with mobility aids like Hoyer lifts or member transfers a plus. Understanding of HIPAA regulations to protect resident privacy and confidentiality

Requirements



High school diploma or GED, or the ability to pass an Educational Proficiency Testing

Valid NYS driver's license (If you do not have one, please note that as some agencies do not require one)

Reliable transportation (you do not need always need your own car)

Ability to provide close supervision and follow detailed support plans

Comfort working around medical equipment (training provided, no clinical responsibility)

Ability to remain calm, attentive, and patient in a medically/behavior involved environment if needed

Willingness to complete required OPWDD self-direction onboarding and documentation

Ability to work independently while respecting family leadership and boundaries

Smartphone and/or tablet for electronic record keeping.
