Duties
Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
Monitor residents' health by observing vital signs and reporting any changes or concerns promptly
Prepare nutritious meals tailored to individual dietary requirements and assist with feeding when necessary
Administer medications following care plans and ensure compliance with HIPAA regulations for patient privacy
Support residents with memory care needs such as Alzheimer's disease or dementia by implementing specialized techniques and maintaining a safe environment
Use assistive devices like Hoyer lifts or resident's to ensure safe transfers and prevent injury during heavy lifting or mobility assistance
Maintain cleanliness of living spaces through regular cleaning duties to promote hygiene and comfort
Document care activities accurately in accordance with care plans and healthcare regulations
Engage residents in social activities and provide companionship to foster emotional well-being
Collaborate with healthcare professionals, social workers, and family members to deliver comprehensive care tailored to individual needs
Follow safety protocols including CPR and first aid procedures to respond effectively during emergencies
Support hospice or palliative care efforts by providing compassionate presence and assistance
Skills
Proven experience in home & community care, assisted living, nursing homes, or group homes setting
Strong knowledge of patient observation, vital signs monitoring, medication administration, and care planning
Familiarity with dementia, Alzheimer's, memory care, autism spectrum disorder (ASD), and working with people with developmental disabilities or disabilities in general
Ability to perform heavy lifting safely using equipment like Hoyer lifts and handle resident transfers confidently
Excellent communication skills for working effectively with clients, families, and healthcare teams
Knowledge of HIPAA regulations ensuring confidentiality and privacy of patient information
Competency in basic caregiving tasks such as cooking nutritious meals, cleaning living areas, and assisting with ADLs
Certifications preferred: CPR, First Aid, home health experience, hospice care training
Experience working in assisted living environments or long-term care facilities is highly valued
