This position will lead the Diabetes Prevention Program (DPP) efforts for the Community and Clinical Connections for Prevention and Health Branch. This position works in a fast-paced environment requiring quick responses to state and national partners and other stakeholders. This position must be flexible as tasks may change over time as the Centers for Disease Control and Prevention provides feedback to annual workplans requiring a shift in the activities being implemented. Current tasks include:

  • Support the state-wide DPP Navigator Referral Service to support provider referrals to enroll participants in a CDC-recognized Diabetes Prevention Program. This includes serving as back up for DPP Navigators if needed. This Service requires high quality customer service as healthcare providers, DPP lifestyle coaches, and referred patients are engaged.
  • Provide technical assistance to DPP sites to include providing online trainings, resources, assistance with establishing or expanding DPPs, and assistance with referring to the DPP Navigator Service.
  • Work with healthcare providers to incorporate the DPP Navigator Referral Service form into the electronic health record to streamline the referral process.
  • Create monthly DiabetesFreeNC newsletters that will be disseminated to lifestyle coaches and program coordinators throughout the state.
  • Develop and host quarterly DiabetesFreeNC webinars for lifestyle coaches and program coordinators throughout the state.
  • Manage the DiabetesFreeNC website including responding to inquiries and updating resources and information.
  • Implement marketing efforts designed to promote DiabetesFreeNC using materials created by the CCCPH Branch and partners. The marketing efforts may include presentations, webinars, toolkits, electronic messages, ads, and newsletters.
  • Assist with evaluation efforts by identifying and collecting qualitative and quantitative data and submitting to the evaluation team to support two cooperative agreements.
  • Leverage existing pharmacy partnerships to develop and host a learning collaborative to serve a network of at least 10 pharmacies who are current CDC-recognized DPPs.
  • Work with existing Community Health Worker (CHW) partners to identify and engage CHWs who are currently or considering delivering the DPP lifestyle change program.
  • Assist with updating the NC CHW Standardized Core Competency training module on diabetes prevention and management.
  • Work with the NC Community Health Worker Association to develop a diabetes specialty training option for NC CHWs seeking re-certification or a diabetes specialist designation.
  • Serve as the DPP content expert providing technical assistance or expertise when needed.

Minimum Education and Experience Requirements
Bachelor\'s degree from an appropriately accredited institution and three (3) years of experience in the program related to the area of assignment;
OR
an equivalent combination of education and experience.
