We're seeking a knowledgeable and compassionate Diabetes Educator to support individuals in managing their health--without the demands of a full-time schedule.

No Diabetes Educator license required.

Monitor My Health is a 501(c)3 non-profit with a mission to provide evidence-based health promotion programs for chronic disease prevention and management in the community, and to address the social determinants of health.

Key Responsibilities:

Education Programs:

Group Sessions: Facilitate group education consisting of 9 sessions to create a supportive environment where individuals with diabetes can share experiences and learn from one another. Group sessions will consist of 10-15 participants, and each cohort will meet weekly for 9 weeks.

Individual Counseling:

Provide one-on-one counseling to patients (up to 20 minutes each), offering personalized guidance on self-management strategies and lifestyle changes.

Monitoring and Evaluation:

Track patients' progress and outcomes, adjusting educational plans as needed. Evaluate the effectiveness of educational programs.

Experience:

Previous experience in diabetes education or a related field is beneficial. Experience working with diverse populations and cultural competence is also desirable.

Qualifications:

Bachelor's

or

Master's degree in a relevant field (e.g., nursing, dietetics, health education). Knowledge of diabetes management principles, including medication, nutrition, physical activity, and psychosocial aspects. Strong communication and interpersonal skills. Ability to work collaboratively in a healthcare team.

Preferred:

Certification as a Diabetes Educator (CDE)

Time Commitment:

Once per week for about 1 hour for one class, and you can hold multiple classes.
